Understanding Eligibility

Our services are available to individuals diagnosed with an intellectual or developmental disability before the age of 22. Through compassionate engagement, we work to understand each person’s unique goals and aspirations, tailoring our support to enhance their quality of life.

Common Diagnoses That May Qualify:

While this is not exhaustive, the following diagnoses are commonly eligible for support services:

  • Intellectual Disability (Mild, Moderate, or Severe; IQ below 70)

  • Autism Spectrum Disorder

  • Cerebral Palsy

  • Epilepsy and Seizure Disorders

  • Down Syndrome

  • Angelman Syndrome

  • Prader-Willi Syndrome

  • Fragile X Syndrome

  • Traumatic Brain Injury (TBI), if diagnosed before age 22

Required Documentation for Eligibility

To determine eligibility, individuals must provide:

  1. Current Medical Examination

  2. Psychological Evaluation

  3. Psycho-social Evaluation
