Community Habilitation
Our Community Habilitation program is designed to support individuals within their private homes, living arrangements, or community settings, with the overarching goal of fostering greater independence and access to community resources. We aim to empower each person to live independently with minimal staff support, enhancing their quality of life and sense of autonomy.
Key Program Elements:
Independence-Focused Life Skills Training: Our team works closely with individuals to develop essential life skills, which may include:
Grocery shopping and meal planning
Cooking and safe food preparation
Laundry and clothing care
Personal hygiene and self-care routines
Home organization and cleaning
Navigating public transportation
Personalized Program Planning: Program plans are developed collaboratively, incorporating input from both the individual and their family. These customized plans outline the desired outcomes and provide a clear path for individuals to work towards increased independence.
Trained, Supportive Staff: Our skilled staff members assist individuals in achieving their goals, providing guidance and encouragement as they practice and master new skills.
Family-Centric Coordination: This service is designed to supplement the care provided by family members and, when appropriate, is coordinated with other services the individual receives. This ensures a holistic approach that aligns with the individual’s full spectrum of support.
Individuals in our Community Habilitation program typically receive several hours of support each week, with services flexibly tailored to meet their specific needs and goals.
